Consent to Participate:

By signing below, I, the undersigned parent/guardian, give my consent for the above-named athlete to participate in the NCS Prospects 360 event(s), including any athletic performance testing, evaluation, and/or any other activities associated with NCS Prospects 360.

 

Consent to Use of Name, Image, and Likeness:

I hereby grant National Championship Sports (NCS) and NCS Prospects 360 the right to use, reproduce, and publish the name, image, and likeness of the above-named athlete in any promotional materials, digital platforms, social media channels, email communications, and other forms of media (print, digital, or otherwise) for the purpose of promoting future NCS events or showcasing the athlete’s participation in NCS events. I acknowledge that these images and likenesses may be used indefinitely and waive any claims for compensation related to the use of such materials.

 

Waiver of Liability:

I acknowledge and understand that participation in any NCS Prospects 360 event involves physical activity that could result in injury. I, the undersigned, agree to release, indemnify, and hold harmless National Championship Sports (NCS), NCS Prospects 360, their directors, staff, contractors, agents, NCS partners, volunteers, and affiliates, from any and all liability for any injury, loss, damage, or expense incurred by the athlete or others, including spectators, during the event or related activities, including travel to and from the event. This includes injuries that occur during, before, or after the event, on or off the field of play, or while participating in any event-related activities.

 

Insurance Coverage Acknowledgment:

I acknowledge that the athlete is covered by my insurance policy, and I affirm that NCS Prospects 360, NCS, and their affiliates, including staff and volunteers, will not be held responsible for any medical or property damage expenses resulting from injuries to the athlete, myself, or any other persons during participation in the event. This includes any damage or injury to personal property (such as vehicles, equipment, or belongings) at the event or while traveling to and from the event.

 

Agreement:

By selecting "Yes, I accept.", I acknowledge that I have read, understood, and agree to the terms outlined in this Consent and Release Form. I further understand that my signature provides full authorization for the participation of the athlete in NCS Prospects 360 events, including the waivers of liability and consent for the use of the athlete’s name, image, and likeness as described.
